Fact Check: PM Modi’s image being printed on death certificates?

Recently, a picture of a man holding a document that has PM Modi’s image on it is going viral on social media
Many criticized PM Modi saying that he seeks publicity even through death certificates. The image is viral on Twitter and Facebook with a similar narrative.

However, a reverse image search done by Altnews on the photograph, directed to a clearer image published in an article by India times on April 17, 2021.
According to the articles, NCP leader from Maharashtra Nawab Malik on April 17 said that the death certificates of COVID-19 victims should also carry the image of PM Modi. Malik said that the prime minister should take responsibility of the fatalities recorded because of the second wave of COVID-19.
In the document Provisional Certificate for COVID-19 Vaccination (1st Dose) can be seen written. The image is of Covid-19 vaccine certificate and not death certificate
